Founded January 1996; incorporated in August 1996 as Christian Community Health Care, Inc.
Founders: Jeff Britton, Steve Zehr, Matthew Heller, MD
Borrowed space from Church of the Good Shepherd in Leo, Indiana, from 1997-2005
Purchased current Grabill location in 2005 from First Care Family Physicians and opened for service in April 2006.
